Introduction

The Sprint Diagnostics Wellness Family Card is designed to provide affordable healthcare benefits for families at a nominal cost of ₹200/-. This card ensures significant discounts on diagnostic services and offers exclusive privileges to promote preventive health.

Objective: To encourage preventive health checkups and make quality diagnostics accessible and affordable for every family.

Eligibility & Membership

  • The card covers one primary cardholder and up to four dependents (total five members).
  • Validity of the card is for the lifetime of the scheme, i.e., until Sprint Diagnostics continues the Wellness Family Card program.
  • The card will be activated within 7 working days from the date of successful payment and submission of complete documents.
  • Aadhaar Card of all members is mandatory for authentication of details (name, age, etc.) to prevent misuse.
  • The application form will be treated as consent for using Aadhaar data only for authentication purposes, ensuring full privacy protection.
  • Membership coverage: The card can include any five individuals nominated by the applicant at the time of registration.

Name should not contain numbers, special characters, or 3+ repeating letters
Please enter a valid email address (only lowercase letters, numbers, and standard email characters)
District should contain only letters and spaces
Pincode must be exactly 6 digits
Aadhaar must be exactly 12 digits

  • 5. 25% discount on Cancer & Tumor Marker Tests.
  • 6. 10% discount on Standard Health Checkup Packages.

Note: Discounts cannot be clubbed with any other promotional offers, coupons, or time-bound packages.

Wellness Family Card Privileges

Card Delivery

  • The Wellness Family Card will be delivered by courier or collected by the applicant at the nearest Sprint Diagnostics center to the registered residential address provided during application.
  • In case of incomplete, incorrect, or invalid address, the courier will be returned to Sprint Diagnostics.
  • Members will be required to collect the card in person from the nearest Sprint Diagnostics branch if delivery fails due to address issues.
  • A re-dispatch fee will apply in cases of repeated delivery failures, and courier charges for re-dispatch will be borne by the applicant.

Terms & Conditions

  • Members must present the Wellness Family Card along with Aadhaar to avail benefits.
  • Discounts are applicable only on standard tariffs and not valid on promotional offers or time-bound packages.
  • Discounts are applicable only on in-house tests. Outsourced tests are excluded.
  • Card can be used at all Sprint Diagnostics company-owned centers. Franchise and satellite centers are excluded.
  • Card details, once processed, cannot be altered or modified.
  • In case of card loss, a re-issue fee of ₹100 will be charged.
  • In case of damage to the card, re-issue charges will apply as per company policy.
  • Costs of contrast materials and radioactive drugs will be borne by the patient at actuals.
  • The card is non-transferable and can be used only by registered members.

Card Ownership & Misuse

  • The Wellness Family Card remains the property of Sprint Diagnostics.
  • In case of misuse, misrepresentation, or false information, Sprint Diagnostics reserves the right to revoke or cancel the card without refund or appeal.

Communication & Updates

Any updates, modifications, or changes to privileges or terms will be communicated through the official Sprint Diagnostics website and notice boards at branches.

Rights of Sprint Diagnostics

Sprint Diagnostics reserves the right to change, alter, modify, or cancel any privileges, benefits, or terms of the Wellness Family Card at any time without prior notice.

General Policy Notes

  • Membership fee once paid is non-refundable, irrespective of card usage.
  • The card is intended for personal healthcare of the registered members, and any misuse will result in cancellation.
  • All disputes, if any, shall be subject to the local jurisdiction of the respective area where the concerned Sprint Diagnostics branch operates.
